Dr. X performed prostate examinations on plaintiff over a period of years, noted a hardened area on his prostate gland, yet failed to refer him to a urologist, causing his prostate cancer to remain undiagnosed and treated until it had metastasized and was not able to be cured. In addition, Dr. X intentionally altered his records in an attempt to hide his findings on prostate examination.
